Sildenafil and tadalafil: questions beyond the name

Start with the treatment schedule and your health history, then discuss which prescription, if any, is appropriate.

What these medicines have in common

Sildenafil and tadalafil belong to the PDE5 inhibitor class. For ED, they help increase blood flow to the penis during sexual stimulation. They do not create sexual desire, cure every cause of ED or provide protection against pregnancy or sexually transmitted infections. That is a useful starting point when an advertisement makes a medicine sound like an answer to every part of sexual wellbeing. [Sildenafil information] [Tadalafil information]

A clinician still needs to consider the reason for the difficulty, your other medicines and your health history. The familiar names do not make either medicine suitable for everyone. The visit preparation page offers a way to gather that information without trying to diagnose yourself.

Discuss the schedule before comparing convenience

For ED, sildenafil is generally prescribed for use before sexual activity. Tadalafil can be prescribed either on an as-needed schedule or as a daily treatment. Those descriptions are not instructions for your own prescription. Ask the prescriber which schedule applies, how it fits your circumstances and what to do if the written directions are unclear. Do not move between schedules or add an extra medicine on your own.

Food is another practical question: the sildenafil information notes that a high-fat meal may delay its effect, whereas tadalafil may be taken with or without food. Explain your ordinary routine rather than trying to select a product solely from a timing chart. A chart cannot predict the response of an individual person or establish a guaranteed window.

Recognize when the product has changed

A generic tablet and a compounded chew, liquid or combination containing the same ingredient are different products. FDA approval of an ingredient in one medicine does not approve every formulation using it. If you are offered a compounded option, ask why an approved product would not meet your medical needs and what evidence supports the specific proposal. Our compounding guide develops those questions. Provider directories can make this distinction easy to miss because one company may sell several categories. The Hims and Ro profiles therefore discuss the product menu before the headline offer. Choosing a company is not the same as choosing a medicine.

Know where routine questions end

Do not combine sildenafil or tadalafil with nitrate medicines or recreational nitrites. Tell the clinician about all other medicines, including riociguat, and any advice to avoid sexual activity for medical reasons. Follow your own prescription label and ask the pharmacist to check uncertainties before use.

An erection lasting longer than four hours needs urgent medical attention. Sudden vision or hearing loss also needs immediate medical attention; do not take further ED medicine before receiving medical advice. Chest pain, fainting or severe breathing difficulty requires emergency help. These are reasons to seek care, rather than wait for a routine refill message. Read the full linked medication information for warnings and precautions; this guide is not an exhaustive safety list.
